Biomedical research is increasingly globalized with ever more research conducted in low and middle-income countries. This trend raises a host of ethical concerns and critiques. While community engagement has been proposed as an ethically important practice for global biomedical research, there is no agreement about what these practices contribute to the ethics of research, or when they are needed.

BackgroundHuman female orgasm is a vexed question in the field while there is credible evidence of cryptic female choice that has many hallmarks of orgasm in other species. Our initial goal was to produce a proof of concept for allowing females to study an aspect of infertility in a home setting, specifically by aligning the study of human infertility and increased fertility with the study of other mammalian fertility. This paper looks at how the strategies of mediaand cultural studies can be applied to thehealth studies field. This relationship,however, has been met with resistance due to anumber of status debates. We argue theimportance of fostering links between these`disciplines' namely because the definition ofwhat constitutes `health' has been broadenedand is inscribed in most forms of popularmedia. As concerns about the negative health effects of unhealthy eating, overweight and obesity have increased, so too have policy efforts to promote healthy eating. Federal, state, and local governments have proposed and implemented a variety of healthy eating policies. Many of these policies are controversial, facing objections that range from the practical (e.g., the policy won’t succeed at improving people’s diets) to the ethical (e.g., the policy is paternalistic or inequitable). State Medical Boards (SMBs) can take severe disciplinary actions (e.g., license revocation or suspension) against physicians who commit egregious wrongdoing in order to protect the public. However, there is noteworthy variability in the extent to which SMBs impose severe disciplinary action. In this manuscript, we present and synthesize a subset of 11 recommendations based on findings from our team’s larger consensus-building project that identified a list of 56 policies and legal provisions SMBs can use to better protect patients from egregious (...) wrongdoing by physicians. This study adapted and tested the efficacy of the Red-Light Purple-Light games for improving executive function skills in preprimary classrooms in Nairobi, Kenya. A cluster randomized controlled trial was used to evaluate the efficacy of the adapted RLPL intervention. Specifically, 24 centers were randomized to the RLPL or a wait-list control condition. Consistent with previous studies, participating classrooms delivered 16 lessons across an 8-week intervention period. A total of 479 children were recruited into the study. After exclusions based on child (...) age and data quality, 451 and 404 children had completed computerized assessments of EF skills at pre- and posttest assessments, respectively. Children in the RLPL centers did not demonstrate any improvements in EF skills relative to their peers in the wait-list control condition. Exploratory tests of moderators were also all null. Results are discussed with respect to measurement limitations and contextual factors that may explain the null results of RLPL on EF skills in young children in Kenya. (shrink)

Over 40 years of research have shown that traumatic brain injury affects brain volume. However, technical and practical limitations made it difficult to detect brain volume abnormalities in patients suffering from chronic effects of mild or moderate traumatic brain injury. This situation improved in 2006 with the FDA clearance of NeuroQuant®, a commercially available, computer-automated software program for measuring MRI brain volume in human subjects. More recent strides were made with the introduction of NeuroGage®, commercially available software that is based (...) on NeuroQuant® and extends its utility in several ways. Studies using these and similar methods have found that most patients with chronic mild or moderate traumatic brain injury have brain volume abnormalities, and several of these studies found—surprisingly—more abnormal enlargement than atrophy. More generally, 102 peer-reviewed studies have supported the reliability and validity of NeuroQuant® and NeuroGage®. Furthermore, this updated version of a previous review addresses whether NeuroQuant® and NeuroGage® meet the Daubert standard for admissibility in court. It concludes that NeuroQuant® and NeuroGage® meet the Daubert standard based on their reliability, validity, and objectivity. Due to the improvements in technology over the years, these brain volumetric techniques are practical and readily available for clinical or forensic use, and thus they are important tools for detecting signs of brain injury. Written by colleagues and friends in the United States and Britain, the essays are intended to reflect Simpson's own legal interests. The collection opens with biography of Simpson's academic life which notes his major contribution to legal thought, and closes with an account of his career in the United States and a (...) bibliography of his writings. As a tribute to Simpson's varied interests in the law, the collection is grouped around themes in human rights, legal philosophy, and legal history. The human rights papers are concerned with the history of the right of individual petition to the European Court of Human Rights, and recent successes in which Brian Simpson played a part; the evolution of a transnational common law of human rights; the United Nations Convention on the Rights of the Child and the interpretation of the provisions on identity in France and England; the suspension of human rights which would have occurred, had the emergency War Zone Courts scheme been brought into effect during wartime; historical resistance to colonial laws in Papua New Guinea; and the ratio decidendi of the story of the Prodigal Son. A central job for propositions is to be the objects of the attitudes. Propositions are the things we doubt, believe and suppose. Some philosophers have thought that propositions are sets of possible worlds. But many have become convinced that such an account individuates propositions too coarsely. This raises the question of how finely propositions should be individuated. An account of how finely propositions should be individuated on which they are individuated very finely is sketched. Objections to the effect that the (...) account individuates propositions too finely are raised and responses to the objections are provided. I give an overview of the trust literature and then of six central issues concerning epistemic trust. The survey of trust zeroes in on the kinds of expectations that trust involves, trust’s characteristic psychology, and what makes trust rational. The discussion of epistemic trust focuses on its role in testimony, the epistemic goods that we trust for, the significance of epistemic trust in contrast to reliance, what makes epistemic trust rational, and epistemic self-trust.

A proof is offered that aims to show that there can be no knowledge of God, excluding knowledge based on natural theology, without divine self-testimony. Both special and general revelation, if they occur, would be forms of divine self-testimony. It is argued that this indicates that the best way to model such knowledge of God is on the basis of an analogy with knowledge gained through testimony, rather than perceptual models of knowledge, such as the prominent model defended by Plantinga. Appropriate causal chains and reliable cognitive processes only seem at best to ensure that a belief proposed for acceptance is the belief the testifier wants accepted; they do not ensure that it is rational to accept the belief. This particularly applies where there is much at stake, where it seems rational to seek some form of evidence, if available. Some brief comments are made on Trinitarian self-testimony. Another model of the ‘inner witness’ is briefly sketched out, based on the analogy with conscience. This model may capture some of the features of Plantinga’s approach, but leaves room for a free rejection of divine self-testimony, in a way that the perceptual analogy does not. A point connected to Plantinga’s aims is then made about the link between evidence, value and divine self-testimony, in relation to religious experience. Finally, it is suggested that the earlier proof may apply in a particular sense to all knowledge of God, including that based on natural theology. (shrink)

Psychedelics such as psilocybin reliably produce significantly altered states of consciousness with a variety of subjectively experienced effects. These include certain changes to perception, cognition, and affect,1 which we refer to here as the acute subjective effects of psychedelics. In recent years, psychedelics such as psilocybin have also shown considerable promise as therapeutic agents when combined with talk therapy, for example, in the treatment of major depression or substance use disorder.2 However, it is currently unclear whether the aforementioned acute subjective (...) effects are necessary to bring about the observed therapeutic effects of psilocybin and other psychedelics. This uncertainty has sparked a lively—though still largely hypothetical—debate on whether psychedelics without subjective effects (“nonsubjective psychedelics” or “non-hallucinogenic psychedelics”) could still have the same therapeutic impact, or whether the acute subjective effects are in fact necessary for this impact to be fully realized.3,4,5. M. Roch Bouchard has contributed an intriguing essay to Idealistic Studies: “Idealist Requirements and the Affirmation of the Other World—the Lachelier Case”. His subject, the distinguished French idealist, Jules Lachelier, is shown by M. Bouchard to have idealized in triumphantly paradoxical form certain tensions between Divine Immanence and Transcendence that have troubled Biblical religionists for millennia. Problems raised for the educated faithful by thinkers like Parmenides, Plotinus, Eriugena, Boehme, Spinoza, Goethe, and Hegel came home all at once to roost for (...) Current methods of obtaining an informed consent leave much to be desired. Patients rarely read consent forms or understand all of the risks, benefits, or alternatives associated with their treatment. Evaluating the advantages and disadvantages of treatment options often presents a more significant challenge for patients with lower levels of health literacy. This article reviews the evidence of shortcomings in our informed consent system and then explores the potential for a new approach to engage patients at all levels of health (...) literacy in their treatment decisions. Specifically, the article will examine the potential of shared decision-making to bridge gaps in knowledge, increase patient adherence to treatment, and improve health outcomes in low health literacy patient populations. Leveling barriers to treatment information for disadvantaged populations should be a public health imperative, especially if it can be shown to improve health outcomes and reduce health disparities. This article seeks to consider the compatibility between the doctrine of the Trinity and the theory of the transcendental properties by offering a consideration of the notion of the ‘gift’ as a transcendental term. In particular, this article presents a re-reading of John Milbank’s influential theology of the gift through Colin Gunton’s project of developing ‘trinitarian transcendentals’. In addition to showing how Milbank’s notion of the gift could be systematically understood in terms of what Gunton calls a ‘trinitarianly developed transcendental’ (...) which nonetheless avoids many of the problems found in Gunton’s original project, this article argues understanding ‘gift’ as a transcendental term can not only provide us with new ways of reconceiving the relationship between the philosophy of transcendental properties and various traditional doctrines, it can moreover show us how the traditional and biblical names of the Holy Spirit as ‘the Gift’ and the Son as ‘the Word’ can offer new ways of developing distinctively trinitarian accounts of metaphysics. Numerous authors identify a white supremacist ideology that shapes the educational opportunities for racially diverse students. We contend that this ideology informs educational policy and hampers the likelihood that racially diverse populations can achieve success at levels similar to students of European descent. In this paper we define the white supremacist ideology as it informs education policy and practices. Three examples from the United States are then used to illustrate the influence of such an ideology. These examples include the creation (...) and protection of racially segregated schooling; desegregation policies; and the current uses of school report cards. We conclude with the relevance of this discussion to educational debates in Great Britain and South Africa, and recommendations to minimise the influence of this ideology on education policy and school reform efforts. Recent discussions of structuralist approaches to scientific theories have stemmed primarily from Worrall's, in which he defends a position whose historical roots he attributes to Poincare. In the renewed debate inspired by Worrall, it is thus not uncommon to find Poincare's name associated with various structuralist positions. However, Poincare's structuralism is deeply entwined with both his conventionalism and his idealism, and in this paper we explore the nature of these dependencies. What comes out in the end is not only a (...) clearer picture of Poincare's position regarding structuralism, but also two arguments for versions of epistemic structuralism different in kind from that given by Worrall. In this chapter, we argue that Du Châtelet’s account of motion is an important contribution to the history of the absolute versus relative motion debate. The arguments we lay out have two main strands. First, we clarify Du Châtelet’s threefold taxonomy of motion, using Musschenbroek as a useful Newtonian foil and showing that the terminological affinity between the two is only apparent. Then, we assess Du Châtelet’s account in light of the conceptual, epistemological, and ontological challenges posed by Newton to (...) any relational theory of motion. What we find is that, although Du Châtelet does not meet all the challenges to their full extent, her account of motion is adequate for the goal of the Principia: determining the true motions in our planetary system. Feminist theology is committed both to listening to the voices of women outside the academy and to challenging all theology that is oppressive to women. This article discusses the tension between these two imperatives when women inside and outside the academy disagree about what is oppressive. The lens through which this is explored is that of differing views about finitude and eternity between feminist academics and working-class women in the north-east of England. Complete moral consensus on the status of the human embryo is neither feasible nor necessary for the formulation of ethically acceptable public policy for human embryo research. Significant consensus on permissible human embryo research can rest upon diverse but overlapping moral traditions. Thus, human embryo research policy should do more than reflect mere abstract assertions about the moral status of human embryos. Rather, the moral underpinnings of human embryo research should be derived from a range of values, including the facilitation (...) of human procreation, the advancement of applied scientific knowledge, the reduction of human suffering, and the protection of vulnerable persons from coercion and exploitation. From 1908 to 1922, William Beebe, the curator of birds at the Bronx Zoo, tried unsuccessfully to bring tropical birds known as hoatzin to the zoological park in the Bronx run by the New York Zoological Society. Beebe was committed to bringing hoatzin to the zoo because he thought they could reveal scientific truths about ecology and evolution to him and the visiting public. While contemporary scholarship about zoo science in the United States has focused on how environmental conservation shaped (...) the scientific practices of zoos at the turn to the 20th century, Beebe's efforts to bring hoatzin to the Bronx shows that evolutionary theory and ecology were central issues at the institution. Beebe wanted to use zoo displays to conduct research into the relationships between organisms and environments in order to investigate how certain animal behaviors, such as flight, developed. Beebe's employers, Madison Grant and Henry Osborn, were less interested in Beebe's experimentation with zoo exhibits, and more invested in using the zoo as a place to disseminate evolutionary ideas they already understood as scientific truths about heredity, racial hierarchies, and eugenics. Whereas Beebe saw his scientific practice as dependent on ecological connections, Grant and Osborn stressed a narrative of purity and separation (of species and race) that governed the conservation projects and scientific focus of the Zoological Society, as well as the presentation of animals on zoo grounds. Ultimately, Grant and Osborn's evolutionary concerns and their vision of a hierarchical natural world dictated the scientific direction of the Bronx Zoo in a way that prevented the institution from becoming a site for ecological research. Irish Travellers constitute a pre-demographic-shift population living among a post-demographic-shift one. Their socio-medico profile identifies them as largely on fast life-history trajectories. In addition, they are strongly religious, highly sexually behaviorally dimorphic, with strong traditions of male-male competition and quasi-symbolic bride capture. Their male-male competitions thus allow for the comparative testing of a number of interesting theories pertaining to the nature and function of types of violence in society. As a pilot study, we used expert raters to analyze a number (...) of real-life bare-knuckle competitions in terms of the support said spectacles offered to theories of this sort of violence as reinforcing ideas of antisociality, hierarchical promotion, intersexual signaling, or maintenance of within-group equality. We found good evidence to support theories of within-group, prosocial hierarchical functions for these contests. Limitations and implications for future research, such as direct measurement of fitness, are discussed. (shrink)

Afferent feedback can appreciably alter the pharyngeal phase of swallow. In order to measure the stability of the swallow motor pattern during several types of alterations in afferent feedback, we assessed swallow during a conventional water challenge in four anesthetized cats, and compared that to swallows induced by fixed (20 Hz) and stochastic (1-20Hz) electrical stimulation applied to the superior laryngeal nerve. The swallow motor patterns were evaluated by electromyographic activity (EMG) of eight muscles, based on their functional significance: laryngeal (...) elevators (mylohyoid, geniohyoid, and thyrohyoid); laryngeal adductor (thyroarytenoid); inferior pharyngeal constrictor (thyropharyngeus); upper esophageal sphincter (cricopharyngeus); and inspiratory activity (parasternal and costal diaphragm). Both the fixed and stochastic electrical stimulation paradigms increased activity of the laryngeal elevators, produced short-term facilitation evidenced by increasing swallow durations over the stimulus period, and conversely inhibited swallow-related diaphragm activity. Both the fixed and stochastic stimulus conditions also increased specific EMG amplitudes, which never occurred with the water challenges. Stochastic stimulation increased swallow excitability, as measured by an increase in the number of swallows produced. Consistent with our previous results, changes in the swallow motor pattern for pairs of muscles were only sometimes correlated with each other. We conclude that alterations in afferent feedback produced particular variations of the swallow motor pattern. We hypothesize that specific SLN feedback might modulate the swallow central pattern generator during aberrant feeding conditions (food/liquid entering the airway), which may protect the airway and serve as potentially important clinical diagnostic indicators. Lots of contextually sensitive expressions appear to have context invariant meanings that do not by themselves suffice to secure semantic values for those expressions in context. For example, suppose I say 1. She is smart. where I do not demonstrate any female, I don’t intend that some female is the semantic value of my use of ‘she’, no female is uniquely salient in the context of utterance, and no female has been under discussion. It would appear in such a case (...) that the context invariant meaning of ‘she’ does not secure a semantic value in context for my use of ‘she’, resulting in infelicity. After all, what would that semantic value in context be? This appears to show that the context invariant meaning of ‘she’ does not by itself secure semantic values in context for it. The class of expressions that are like ‘she’ in this respect is quite large. It arguably includes simple and complex demonstratives, tense, expressions taking implicit arguments, gradable adjectives, quantifiers, ‘only’, possessives, conditionals, modals and more. I call these expressions supplementives to highlight the fact that their context invariant meanings need to be supplemented in context for them to secure semantic values in context. I claim that supplementives differ from each other in the following two ways: 1. The degree to which normal speakers are explicitly aware that the expression is contextually sensitive. 2. The degree to which ordinary speakers are explicitly aware of what sort of semantic value in context the expression takes. I hold the view that semantic values in context for supplementives are fixed by recognizable speaker intentions. However, I argue that given the differences between supplementives with respect to 1 and 2, the intentions fixing the semantic values in context of supplementives that differ with respect to 1 and 2 will themselves be different, while still all being speaker intentions that some entity o be the semantic value of the use of the supplementive in context. For those who retain a post-orthodox religious temperament in post-modernity, Robert S. Corrington’s evolving account of Ecstatic Naturalism might prove a challenging, engaging framework for a transcendental naturalism. If one can read Emerson and Thoreau and ignore their religious dimension, so be it—attunement is crucial for Corrington, cultivating the habits of thought, the (...) psychosemiosis that enables an engagement with Ecstatic Naturalism and a recovery of the sacred.
